Summary: On January 28, 2019 “Orange2fly” operated ACMI Flight OMS 104 on behalf of Salam Air from OMDB to OOMS. The Aircraft departed Dubai OODB at 18:34 UTC, the Commander CM1 was PM and the First Officer CM2 was Pilot Flying PF. At the end of initial approach, being radar vectored Flaps 1 was selected. At 2260 ft IAS 186kts the Commander disconnected the Auto pilot and took over control of the Aircraft. During right turn of the A/C on final at 1840 ft the Commander selected idle thrust and the auto thrust was disconnected. On final the controls of the A/C were handed over to the F/O. With auto thrust disconnected the speed of the A/C was decreasing and at 290 ft (256 ft RA) with IAS 116 kts, TOGA applied and at the same time Alpha Floor Activated. Minimum recorded altitude was 210 ft. The Aircraft recovered and landed at Muscat OOMS at 19:11 UTC. The A/C was landed normally by the Commander.

While performing a visual departure from R/W 23L in the direction of XERIS, the Crew received a TCAS (TA, RA) during initial climb and manoeuvre to avoid a possible collision with a Helicopter passing through the CTR of Kavala Airport and then performed a manoeuvre to avoid a mountain mass. The Aircraft then resumed its flight and landed at its final destination.

Summary: On Friday 29.09.2017, the aircraft owned and operated by "EASYJET Airline Company Ltd." with registration G-EZOC, during a scheduled flight from Mykonos International Airport to Milan Malpensa International Airport and while in Greek airspace, the pilots noticed the smell of burnt plastic and smoke in the cockpit, obviously originating from underneath the first officer's rudder pedals. A forced landing was decided by the flight crew that took place at Athens International Airport.

The Air Accident Investigation and Aviation Safety Board was informed on the same day and with the AAIASB/3112/02.10.2017 document, an investigation team was appointed. On 03.10.2017 a notification was sent to the International Authorities and ACCREP’s were appointed.

Summary: On 21/4/2018, at 12:00 h, the Aircraft Piper PA-28-140 with registration SX-SEX took off from Messolonghi LF with four Pilots onboard with destination Kopaida "Dimitra" LF. All on board were Pilot members of the Araxos Αir Club. Two Pilots would disembark at Kopaida LF in order to pick up another ultralight Aircraft with registration SX-UBX. At 15:20LT SX-SEX Aircraft took off from Kopaida LF with two Pilots on board to return to Messolonghi LF. At 12:57:52LT at an altitude of 2500 ft and while the Aircraft had passed the IXONI point, the pilot reported to the FIC an emergency due to a problem with the fuel pump and his intention to land on the old highway. During the approach for the forced landing, the Aircraft collided with the horizontal support cable between two electricity pillars, resulting in its impact on the road of the old highway where it stopped after traveling a distance of 64.5 m. The forced landing was monitored by the two Pilots of the other Aircraft who informed the FIC and NHERC. The rescue teams that arrived rescued one fatally injured and one injured. AAIASB was informed about the accident and the investigation team was appointed on the same day, while on 23/4/2018 the AAIASB informed the State of design and manufacture of the Aircraft, the European Commission, the European Aviation Safety Agency, the Civil Aviation Authority and the Safety Incidents Reporting Committee.
