Summary: Following the completion of the refueling procedure on the aircraft with registration EI-FRO, fuel company personnel walked up the aircraft forward stair in order to have the relevant fuel form signed and take a copy of such signed form.
Upon the opening of the aircraft door, the door hit him causing him to fall on the ground from a height of about three meters and sustain serious injuries.

Summary: On April the 22nd, 2017 the DIAMOND DA42NG aircraft with registration SX-BEO with two (2) persons on board took off from RWY 05R of Kavala airport (LGKV) in order to perform a scheduled training session on asymmetric circuits as well as touch and goes. During the first circuit, the aircraft landed with the landing gears up and after having dragged a few meters, it finally stopped on the runway. The two persons aborted with no injuries. The aircraft was seriously damaged.

Summary: On Friday 23/06/2017, the helicopter with registration SX-HBB owned by ‘ETHNIKI LEASING S.A.’ and operated by ‘AIR 1 HELICOPTERS’ was situated at the wetland of Schinias – Marathonas, performing aerial spraying operations for the control of mosquitoes, on behalf of the joint venture that had been awarded a contract by Attica Region Authority, for the project of mosquito control in the Regional Unit of Eastern Attica. Following the completion of the scheduled aerial spraying operations, the pilot performed one more flight above the wetland of Schinias with two other persons onboard. During that flight, the helicopter crashed in the wetland, having struck power lines, which resulted in the fatal injury of the pilot and one of the two helicopter occupants, whereas the other person onboard was seriously injured.

Summary: On June 13th, 2015 a Piper PA-46-310P aircraft, 8 minutes after take-off from Araxos (LGRX) airport during climb, it lost approximately 90% of engine power.
The pilot who was the only person on board, tried to land at Mesologgi airfield. The aircraft finally landed on a mad crop field at the end of the runway. There were no injuries.

AAIASB was notified at the same day and on June 15th, 2015 an Investigation Team and a Technical Adviser were designated. On July 12th, 2016 a new member was added in the Investigation Team.

Summary: On October 16th , 2016 at 16:25 h, a single engine airplane Cessna 172P, with registration marks 5B-CLB, owned by Aeroclub of West Greece, took off from Megara airport for a training flight. Destinating airport of the airplane was Megara airport as well. On board the airplane were the instructor and the trainee. According to the flight plan, the airplane was heading towards Kalavryta area.
After an hour of flight, at 17:26, the airplane crashed over the mountains of Kalavryta area at an elevation of 4,965 ft. The airplane was totally destroyed from impact and a post impact fire. Both people on board were fatally injured.

Summary: The amphibious Cessna U206F with registration N195GG attempted unsuccessfully to take off from Mandraki Bay of Hydra island with destination the area of Agios Theodoros of Korinthos. After several attempts without being airborne, it continued its course as a boat. During its approach to the harbor of Agios Georgios near Methana, a sudden wave from the rear, turned the aircraft in an upside down position. Immediately nearby boats reached the amphibious and towed it to the harbor. Due to the shallow water, it was decided to move the aircraft to the harbor of Methana using an Hellenic Coast Guard boat. During this transportation, the rope was broken and the amphibious was sunk.

Summary: On Wednesday the 9th of December 2015 a CESSNA 172M aircraft with registration SX-ACO with three (3) persons on board departed from Arachthos airfield with destination the airport of Megara (LGMG). Reaching Megara ATZ at a height of 1500 ft and 2,5 nm S/SW of the airport, the above mentioned aircraft lost separation and came at a close distance with a PIPER PA28-161 aircraft with registration SX-ARA with two (2) persons on board which has departed from Megara airport. The first aircraft performed a RH manoeuvre in order to avoid the collision with the second aircraft.

Summary: On Sunday the 17th of August 2017 a PIPER PA31 NAVAJO aircraft with registration G-IMEC departed from the State Airport of Syros Island (LGSO) at 14:45 h with destination the airport of Pachi Megara (LGMG). At destination airport there were strong winds and during landing a sudden gust made the aircraft performing a long flare. As the plane touched the ground the available runway length was insufficient for a safe landing, so the pilot applied too much force on the brakes and the LH tire burst. The pilot realized that he could not stop the aircraft on the runway so he turned left towards taxiway “A” and because of its high speed the aircraft finally veered. The RH landing gear collapsed and the RH wing touched the ground. The aircraft stopped in the middle of taxiway and the passengers aborted with no injuries. No fire occurred.

Summary: On 2 February 2015, the Jetstream 41 operated by “SKY EXPRESS”, Registration SX-DIA, was on the scheduled flight ‘SEH 100’ from the Airport of Heraklion ‘N. Kazantzakis’ (LGIR) to the Airport of Rhodes ‘Diagoras’ (LGRP). Upon landing in runway 07 of the aerodrome, at 07:36 hrs. local time, while the aircraft was decelerating following touchdown, the left main landing gear collapsed and folded rearward. As a result, the aircraft dragged the runway on its left side and came to rest at the runway left edge and without leaving the runway, about halfway of the runway’s length and facing eastward. There was no fire or injuries, and the passengers were disembarked without any problem.
The Air Accident Investigation and Aviation Safety Board was notified of the accident by means of document Sky Express/3271/02.02.2015 and an Investigation Team was appointed by virtue of document AAIASB/205/02.02.2015.
The competent International and National Authorities were notified under document AAIASB/209/02.02.2015. The state of manufacturer appointed accredited representative (non-travel).

Summary: On 29/12/2015, 16:55 LT, the Ultralight Aircraft (ULA) Trike Apollo Halley type Delta Jet with Registration SX-UAO, took off from Mesologi Airfield with a pilot and one passenger on board, to execute a flight round Mesologi lagoon and to return back for landing at the same airfield.
During flight at an altitude of 1,900 ft over the city of Mesologi, the pilot tried to increase the engine power by pressing the acceleration pedal without having any response. When the pilot released the acceleration pedal the engine power decreased abruptly without been increased again, after many attempts by the pilot, the ULA started losing altitude abruptly.
The pilot directed the ULA for forced landing at the area of old saline across Tourlida area. During forced landing phase, the ULA hit unpowered electricity distribution cables that were transversely crossing a naval channel and crashed inside it.
The two person onboard the ULA were injured and were hospitalized at Mesologi General Hospital.
The AAIASB was informed by Port Authority of Mesologi on the same day, on 30/12/2015 an investigation team was set up and on 05/01/2016 the accident notification was made by the ULA pilot.

Summary: The Cessna 172N aircraft with registration number HA-TAH, with three (3) passengers [one (1) instructor and two (2) trainees] was flying from the Αirfield of Kilkis / Neas Kavalas Polykastrou of the municipality of Paionias to the International Airport of Thessaloniki. During the flight and while the instructor had the controls, the aircraft hit aerial power lines of the Public Power Corporation. Immediately after the hit, the aircraft crashed to a nearby islet within the river Axios, about 3 NM from the Airfield of Neas Kavalas, near the village Rizia of the municipality of Paionias, Kilkis. All three occupants of the airplane were injured.
